Fill out both registration forms, sign and date and leave forms in the KYDS box in the main office.
REGISTRATION IS REQUIRED
You will need to download Adobe Reader. It's Free to download. Once you download Adobe Reader, open the pdf through Adobe and you will be able to write in the pdf. Please have a parent or guardian fill this out.
