OPTION 1: *
- Download 2 fillable registration forms
- Sign and date
- Email both forms as an attachment or clear picture to [email protected]
* You will need to download Adobe reader. It's Free to download.
Once you download Adobe Reader, open pdf through Adobe and
you will be able to write on pdf. Please have a parent/guardian
fill this out.
OPTION 2:
- Pick up registration forms in MAIN OFFICE
- Fill out both registration forms
- Sign and date
- Drop off in KYDS box in the MAIN OFFICE
